Community Health Needs Assessment
Bozeman Health, Gallatin City-County Health Department, HRDC, and One Health/CHP are conducting their newest Community Health Needs Assessment. This survey gathers information on the health and well-being of our community along with prioritizing what the biggest health issues are. This is completely anonymous and takes only a few minutes. Get involved!
Want to be a part of the change? Want to have a voice in the conversation? Here’s how you can help! We are looking for key leaders, coalition members, and sponsors. Below is an organizational chart of the structure of the board and some definitions of what each role may entail.
Key Leaders
Do you have influence in the community and agree that it’s time to prevent youth behavioral health problems before they start? Key leaders can also be a coalition, but do not have to be. They will:
• Support the child well-being and healthy youth development in our community
• Promote the Communities That Care process by informing others about this evidenced-based work
• Help secure needed information and resources for implementing the community’s chosen evidenced-based programs, policies, and practices
• Attend CTC Key Leaders’ Meetings (2x a year)
Coalition Members
If you want to be the boots on the ground, this is the place for you. Coalition members are the people in the community who want to actively be a part of the change and decision-making for our youth. Members will:
•Have access to key stakeholders
• Lead & participate on work groups
• Lead and participate in collective decision-making
• Develop community action plan
• Assist in carrying out community action plan
Sponsor
While some start-up money is provided to a county trying to implement the CTC framework, it is important to ensure sustainability within our efforts! Funding can be general towards the CTC implementation, or it can be more specific. Here are some examples of funding that would greatly help out:
Event/workshop sponsorship
Program purchase
Awareness campaigns
Youth scholarships
